Tora-san comes back to his old home in Shibamata. His university professor Mr Okakura (Masakane Yonekura) suddenly arrives at his home which makes Tora-san so angry that he plans to leave home. However, his old friend Chiyo (Kaoru Yachigusa) comes to visit him. The two of them begins to reminisce about old times, and has developed a kind of bonding between them. At the same time, Mr Okakura has become attracted to Chiyo, which makes this the first ever love triangle for Tora-san. Superstar Kinuyo Tanaka has a special appearance in this film.。外科醫生九十九勝己（三浦貴大）因醫療事故被逐出醫院，走投無路下，透過恩師的介紹來到神酒診所。位於高級住宅區的神酒診所，二樓以上是設備精良的醫療環境，由院長神酒章一郎（安藤政信）率領。醫術精湛的密醫，為需要隱藏病情的大人物們治療。面試那天，勝己目睹章一郎高超的執刀技巧大受震撼，決定要在神酒診所為拯救人命盡最大努力。但事實上，神酒診所暗地裡還提供其他服務，勝己不得不捲進事件當中，也發現自己的醫療事故另有隱情……。
